Athletic Physicals - May 28th

GlenOak Sports Medicine will be offering athletic physicals at GlenOak High School on May 28th at 2:30 PM. The exams will be in the high school athletic training clinic (W115).
- Please be in line by 4:30 p.m. to ensure you receive a physical.
- The cost is $10.00, check or cash will be accepted. Checks payable to GlenOak Athletics.
- Please bring your signed physical paperwork from FinalForms with you. Instructions on how to print the document can be found here(Opens in a new window).
- Please enter through Door 17 and walk down the stairs. Turn right at the bottom of the stairs and proceed through the W-hallway until you reach W-115.
